For: Each 40 mg/dL reduction in LDL-C cuts major cardiovascular events by ~22% (CTT meta-analysis). JUPITER Trial (rosuvastatin), NEJM 2008 showed benefit even at LDL levels once considered "normal."
Against: Statins reduce cardiovascular events even in lower-risk groups, but absolute benefit is smaller — side effects, cost, and personal risk tolerance matter. A "statin vacation" may be reasonable in select low near-term risk cases.
Do I need a CAC scan?
A Coronary Artery Calcium (CAC) score is simpler, cheaper, lower radiation, and more widely available than CT angiography (CCTA) — both involve some radiation exposure. It's most useful when you're in the "gray zone" of risk.
For: If elevated Lp(a), asymptomatic status, and a gray-zone risk profile, ACC/AHA guidelines suggest a CAC score is a reasonable next step to reclassify risk and inform the statin decision.
Against: If my CAC comes back 0, can we defer the statin discussion for ~5 years? A CAC = 0 is reassuring but doesn't fully offset a strongly elevated Lp(a).
